On or about May 18, 2023, and times prior thereto, extension ladder(s) utilized by employees had structural defects and physical damage and were still in service, exposing employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about May 18, 2023, and times prior thereto, employee(s) were performing residential roofing activities at heights of approximately 12 feet and greater without the use of a fall protection system, exposing employee(s) to fall hazards. Juan Ramirez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1663656, citation number One, item number 3 and was affirmed as a final order on May 16, 2023, with respect to a workplace located at 3355 Monarch Pass Drive in Arlington, TX.
