29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a. Envases Media, Inc., at a worksite located at 1 Toy St. Suite 2, Bryan, Ohio 43506: On or about May 22, 2023,  the employer did not ensure energy control procedures for the "assembly machine" were adequately developed and utilized to ensure control of all hazardous energy sources within the assembly machine cell.  A maintenance employee performing adjustments in the cell with other components, such as but not limited to, conveyor 23 in energized state,  were exposed to caught-in hazards.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a. Envases Media, Inc., at a worksite located at 1 Toy St. Suite 2, Bryan, Ohio 43506: On or about May 22, 2023,  the employer did not ensure periodic audits of the energy control procedure were completed at least annually.  Employees were tasked to perform servicing and maintenance activities on the "Assembly Machine" utilizing energy control procedures that have not been reviewed for deficiencies, there by exposing employees to caught-in  hazards.

29 CFR  1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:   a. Envases Media, Inc., at a worksite located at 1 Toy St. Suite 2, Bryan, Ohio 43506: On or about May 22, 2023, The employer did not ensure that maintenance (authorized) employees tasked with service and maintenance activities at or near energized parts inside the Assembly Machine - Cell were adequately trained in the application of lockout procedures in which isolate and control hazardous energy sources during both maintenance and testing tasks. Employees not effectively trained on energy isolation application and testing procedures were exposed to energized components of the cell.
