29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.   a) Throughout the warehouse- On or about June 1, 2023, and at times prior, the employer exposed employees to struck-by hazards in that, the employer did not ensure powered industrial truck operators were provided with training and evaluation prior to operating forklifts.

The bulk oxygen storage location shall be permanently placarded to indicate: "OXYGEN - NO SMOKING - NO OPEN FLAMES", or an equivalent warning.  a) On or about June 1, 2023, the employer exposed employees to struck-by and fire hazards, in that the employer did not ensure oxygen cylinders were stored correctly and away from cigarette smoking areas.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  At Richmond Supply Company, Augusta, GA - On or about June 1, 2023, employees in the warehouse were exposed to hazardous chemical products such as but not limited to Methyl ethyl ketone(MEK) and Acetone. Employees were exposed to chemical hazards, in that, there was no training or chemical information provided.
