Employee(s) are exposed to fall hazards.  Employee(s) are exposed to fall hazards at 2933 NE Kansas Trail Topeka, Kansas.  Employee(s) were securing a metal roof onto a garage structure up to 21.2 feet feet from the top of the roof to the ground below.  Fall protection was not utilized.

Employee(s) are exposed to fall hazards at 2933 NE Kansas Trail, Topeka, Kansas.   Employee(s) were lifted in and working from a work platform on a Forklift 4000 forklift that was not approved by the manufacturer.   Employees were exposed to a fall hazard of over 12 feet from the elevated work platform to the ground below.
