29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about June 7, 2023, Roberto Torres did not ensure that employees working above 6 feet and roofing a steep roof were protected from falling by the use of fall protection in a worksite at 8749 Watson Road in Saint Louis, Missouri. Employees wore harnesses, but they were not tied off to the structure.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  On or about June 7, 2023, Roberto Torres did not ensure that a ladder, which was used to access the second level roof from the first level roof, extended at least 3 feet above the edge of the roof in a worksite at 8749 Watson Road in Saint Louis, Missouri.
