29 CFR  1910.147(f)(1)(ii):When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2):  a) At the R 28 PAN SYS on the site located at 3355 West Memorial Boulevard, Lakeland, FL: On or about 06/08/2023, the employer exposed employees to amputation hazards, in that, conveyor belt guards were removed in order to adjust the belt and employees were not protected or removed from the hazards of belt nip points.
