29 CFR 1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). (a)On or about June 14, 2023, at a jobsite located at 2225 W Mill Rd Glendale, WI 53209, employees working on a new build apartment complex were operating all-terrain forklifts without being properly trained, exposing employees to struck-by hazards.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about June 14, 2023, at a jobsite located at 2225 W Mill Rd Glendale, WI 53209, employees conducting framing work on the roof of a new apartment complex were working without the use of fall protection, exposing employees to an approximate 21 foot fall to the ground.

29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. (a) On or about June 14, 2023, at a jobsite located at 2225 W Mill Rd Glendale, WI 53209, employees conducting sheathing work from a platform supported by an all-terrain forklift were not protected from falling, exposing them to an approximately 21 foot fall hazard.
