At 5319 Shreve Ave, St Louis, MO, On June 14 and at times prior thereto,  Employees were exposed to tripping and struck by hazards from having to access a grate 1.2 feet below the level of a set of conveyor rollers, and then walk over the rollers.

At 5319 Shreve Ave, St Louis, MO, On June 14 and at times prior thereto,  Employees were exposed to fall hazards while working on a conveyor system that was 4.2 feet above a concrete surface without guardrail system, safety net system, or personal fall arrest system.

OSH ACT of 1970 Section (5)(a)(1)  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  A beam was positioned upright on a steel rack that was on a conveyor system.  The conveyor was approximately 50 inches above the floor.  The beam fell off the rack and conveyor onto the floor.    At 5319 Shreve Ave., in St. Louis, MO:  On or about June 14, 2023, and at times prior thereto employees were exposed to struck-by and caught -in-between hazards A beam was positioned upright on a steel rack that was on a conveyor system.
