29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location - At the edge of the dwelling.  On or about 6/16/2023, employees were not protected from potential falls greater than 6 feet to a lower level while accessing/egressing an extension ladder. The ladder did not extend at least feet above the upper landing.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location - The two roofs.  On or about 6/16/2023, employees were not protected from falling to a lower level while working at heights greater than 6 feet in these instances:  a) Employees located on a second level roof.  b) Employees located on a third level roof.  The Four Brothers Construction, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1619822, citation number 1, item number 1 and was affirmed as a final order on 9/30/2022, with respect to a workplace located at 8 Ashland Street, Somerville, MA.  The Four Brothers Construction, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1489241, citation number 1, item number 2 and was affirmed with a final order on 11/4/2020, with respect to a workplace located at 19 York Street, Boston, MA.
