Location: a) Worksite; 27th floor surface; roof (canopy decking) of the amenities area at about the 28th floor a) On or about 6/20/2023, An employee walked/worked near the exposed edge of the amenities area roof/canopy decking, which was approximately 15 feet above the concrete surface of the 27th floor, to perform layout work without the use of fall protection.

Location: a) Worksite; 27th floor surface; roof (canopy decking) of the amenities area at about the 28th floor a) On or about 6/20/2023, the employer did not ensure that employee(s) were protected from falling through a hole by using personal fall arrest systems, covers, and/or guardrails. An employee fell approximately 15 feet through a hole on the roof/canopy decking of the amenities area to the concrete surface of the 27th floor below.
