29 CFR 1910.23(b)(9): (9) Ladders are were not inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury;   A) Richards Plumbing and Heating Co. Inc. - 2800 Victory Blvd. Staten Island NY 10314 - The employer did not ensure ladders were inspected before initial use to check for defects and protect employees accessing a storm basin.  The ladder rung and concrete were deteriorated which led to rung failure and caused an employee to fall approx. 16'9"   Incident occurred on May 18, 2023.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer failed to report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  A) Richards Plumbing and Heating Co. Inc. - 2800 Victory Blvd. Staten Island NY 10314 - An employee fell 16'9" down a manhole and was hospitalized on May 18, 2023. The employer failed to report within the 24-hour time period allotted. Employer reported the  incident on May 22, 2023.  Incident occurred on May 18, 2023.
