29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  (a) On June 28, 2023 and times prior thereto; employees were exposed to shock hazards while using equipment powered by a spider box which was powered by an extension cord. The extension cords outer insulation was pulled away from the plug connector.

29 CFR  1926.432(a)(1)(i):Conductors or equipment were, located in damp or wet locations and were not identified for use in the operating environment:  (a) South end of building: On June 28, 2023 and times prior thereto; employees were exposed to shock hazards while using the Hilti Rotary Hammer and the vacuum which was plugged into the Yellow Jacket relocatable power tap. The power tap was near the water faucet and sitting on damp ground.
