29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Jobsite located at 224 W. 14th St., Wilmington, DE 19801, Roof of Front Porch Duplex: On or about July 12, 2023, an employee working on the roof was exposed to struck by hazards while handing materials and tools up to another employee working from a ladder jack scaffold. The employee working below was not utilizing a protective helmet to protect them from the possibility of impact or falling objects.

29 CFR 1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) 224 W. 14th St., Wilmington, DE 19801, Roof of Front Porch: On or about July 12, 2023, an employee was conducting roofing activities, such as front dormer repair on a ladder jack scaffold without the use of a personal fall arrest system. The employee was exposed to falls up to 23 feet above the next lower level.

29 CFR 1926.451(b)(5)(ii):The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping or has guardrails which block employee access to the cantilevered end.  a) Jobsite located at 224 W. 14th St., Wilmington, DE 19801, Roof of Front Porch Duplex : On or about July 12, 2023, employees were conducting roofing activities, such as front dormer repair on a ladder jack scaffold. The platform of the ladder jack scaffold extended over it supports more than 18 inches exposing employees to falls up to 23 feet lower level.
