29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: On or about June 13, 2023, and at times prior thereto, at PolyP2, Train 3, Area 13, Structure 1B, Landing Elevation 74, Formosa Plastics Plant Site, employees were exposed to 20-foot falls when a steel grating floor opening was not covered or barricaded.
