29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) On or about July 17, 2023, employees were exposed to fall hazards of 12 feet when working on a flat roof where holes were not covered and no other fall protection was in place.  An employee fell through a hole that had been cut for an HVAC unit as a result of this exposure.
