29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about July 11, 2023, the employer did not establish an energy control program for equipment in their facility such as, but not limited to, the Dubois manufactured roll coater machine located in the production area. Employees were exposed to caught-in and amputation hazards associated with ingoing nip points and rotating parts while performing service and maintenance activities including, but not limited to, cleaning.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about July 11, 2023, the employer did not ensure that the Dubois manufactured roll coater machine, located at the production area, was properly guarded on the feed side of the machine to prevent employees from getting their hands and fingers caught-in rotating parts during normal operating cycles. As a result, operators on the feed side of the machine were exposed to caught-in and amputation hazards associated with ingoing nip points and rotating parts.   b) On or about July 11, 2023, located at the production area, the employer did not ensure that the conveyer belt on the ejection end of the Dubois manufactured roll coater machine was properly guarded. Employees were exposed to an ingoing nip point where the conveyer belt wrapped around the drive roller, exposing them to caught-in hazards.  Abatement documentation is required for this item in accordance with 29 CFR 1904.19(d).
