On or about July 14, 2023, where employees perform tree trimming activities from heights in trees, the employer did not ensure anchor points, such as but not limited to the entire tree and the base/roots as an anchor could support at least 5000 pounds exposing employees to falls and struck-by hazards.

29 CFR 1910.140(c)(13)(ii): Anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two. On or about July 14, 2023, where employees perform tree trimming activities from heights in trees, the employer did not ensure anchor points, such as but not limited to the entire tree and the base/roots as an anchor, were designed, and used under the supervision of a qualified person exposing the employee to fall hazards.
