29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  a. At 2000 Estero Blvd, Fort Myers Beach, FL: On or about 07/25/2023, an employee/electrician was exposed to an electrocution hazard, in that, the employee was working with his hands within a underground  electrical enclosure , that which contained energized electrical wires, during which time the employee who was not wearing electrically insulated rubber gloves and who was using an uninsulated hand tool (cutting pliers), attempted to remove the electrical insulation from the end of one of the previously existing energized (red) wires.  b. At 2000 Estero Blvd, Fort Myers Beach, Florida, on or about 07/25/2023, an employee, who was not a qualified electrician was exposed to an electrocution hazard, in that, the employee was working with his  hands pulling wires within/through a underground electrical enclosure, that which contained previously existing energized electrical wires, during which time the employee was not wearing electrically  insulated rubber gloves.

29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:   a. At 2000 Estero Blvd, Fort Myers Beach, Florida, on or about 07/25/2023, an employee/electrician was exposed to an electrocution hazard, in that, the employee was working with his hands within a underground electrical enclosure, that which contained energized electrical wires, during which time the employee who was not wearing electrically insulated rubber gloves and who was using an uninsulated hand tool (cutting pliers), attempted to remove the electrical insulation from the end of one of the previously existing energized (red) wires.  b. 29 CFR 1926.416(a)(3): Before work began the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring a person, tool, or machine into physical or electrical contact with the energized electric power circuit:  a. At 2000 Estero Blvd, Fort Myers Beach, Florida, on or about 07/25/2023, an employee working within a below grade electrical junction box was exposed to an electrocution hazard, in that, before work began the employee did not determine whether the uninsulated hand tool (cutting pliers) could have come into physical or electrical contact with an energized electrical circuit.

At 2000 Estero Blvd, Fort Myers Beach, Florida, on or about 07/25/2023, an employee/electrician was exposed to an electrocution hazard, in that, the employee was working with his hands within a underground electrical enclosure , that which contained energized electrical wires, during which time the circuit breakers had not been Locked-Out or Tagged-Out, and wherein, the employee who was not wearing electrically insulated rubber gloves and who was using an uninsulated hand tool (cutting pliers), attempted to remove the electrical insulation from the end of one of the previously existing energized (red) wires.  b.
