29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  a- 691 Wyckoff Ave, Mahwah NJ 07430 (8 on 12 pitch Residential Roof): CSHO observed employees engaged in roofing activities on an 8 on 12 pitch roof in which a means of fall protection was not being utilized exposing the employees to fall hazards of approximately 25ft to the asphalt below. Observed on or about 07/26/2023.  b-691 Wyckoff Ave, Mahwah NJ 07430 (16 on 12 Residential Roof): CSHO observed employees engaged in roofing activities on a 16 on 12 pitch roof in which a means of fall protection was not being utilized exposing the employees to fall hazards of approximately 25ft to the asphalt below.   c-691 Wyckoff Ave, Mahwah NJ 07430 (Lower Residential Roof): CSHO observed employees engaged in roofing activities on a 8 on 12 pitch lower roof in which a means of fall protection was not being utilized exposing the employees to fall hazards of approximately 15ft to the asphalt below.
