29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a) Lot #185, front side scaffold of new residential structure: the employer exposed employees to struck-by hazards while cutting brick with a DeWalt Chop Saw, in that protective eyewear was not provided to employees engaged in masonry brick cutting activities.

29 CFR  1926.300(b)(1):Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  a) Lot 185, front scaffold: the employer exposed employees to amputation hazards while operating DeWalt (Model D28715) Chop Saw, in that the guard was not installed on the saw while engaged in brick cutting activities.   b) Lot 185, front side of new residential structure: the employer exposed employees to severe laceration/amputation hazards while starting the trailer cement mixer, in that the starter recoil cover/guard was not installed on the engine of the mixer which employees used a nylon strap to pull start.
