This was most recently documented at a worksite located at 511 N. 20th St, Dakota City, Nebraska. The employer failed to ensure that electrical cords were only used for temporary purposes in that an extension cord was attached to a platform that employees were accessing.

The employer failed to ensure that the "EXIT" sign located in the Finished Goods room was illuminated.
