Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from hazards associated with struck by hazards: a. Warehouse- Employees loading, unloading, and working around Industrial steel storage racks with the use of powered industrial trucks were not protected from struck by hazards where the employer did not identify and post the capacities of the steel storage racks, remove damaged wood pallets from steel storage racks that do not have pallet supports, remove damaged steel storage racks from service, and provide a storage rack design professional to evaluate damaged storage rack components. b. Fire Hose Test Area- Employees leak testing fire hoses with couplings were not protected from struck by hazards where the employer did not provide for a positive means of securing the fire hose, coupling, and end cap in place while leak testing procedures are conducted.
