29 CFR 1910.134(d)(2)(i):The employer did not provide either a full facepiece pressure demand SCBA certified by NIOSH for a minimum service life of thirty minutes or a combination full facepiece pressure demand supplied-air respirator with auxiliary self-contained air supply. Employees were exposed to an asphyxiation hazard in that the employer did not provide employees with respirators for use in IDLH atmospheres and oxygen deficient atmospheres in and around the Cryogenic Deflasher machines. Employees were exposed to oxygen deficient atmospheres while performing tasks including but not limited to "Pre-Startup Cleanout" of Cryogenic Deflasher machines.

29 CFR 1910.134(d)(2)(iii):When the atmosphere was oxygen deficient, it was not considered to be IDLH. Employees were exposed to asphyxiation hazards in that the employer did not assess or treat Cryogenic Deflasher workspaces, as immediately dangerous to life and health (IDLH) in the presence of expected, but unmeasured, oxygen-deficient atmospheres. The employer did not implement elements of the respiratory protection standard required for IDLH work exposures. Employees were exposed to oxygen-deficient atmospheres while performing tasks including but not limited to "Pre-Startup Cleanout" of Cryogenic Deflasher machines.

29 CFR 1910.134(g)(3): For all IDLH atmospheres, the employer did not ensure that the requirements of 29 CFR 1910.134(g)(3)(i) - (vi) were met. Employees were exposed to an asphyxiation hazard in that the employer did not treat oxygen deficient atmospheres in and around the Cryogenic Deflasher machines as immediately dangerous to life and health (IDLH). The employer did not implement the required elements of the respiratory protection standard where employees were exposed to IDLH atmospheres while performing tasks including but not limited to "Pre-Startup Cleanout" of Cryogenic Deflasher machines. The employer failed to ensure one or more employee is outside the IDLH atmosphere during entry, determine and implement a system of communication, ensure the employee outside IDLH space is trained in emergency rescue, designate a person to be notified before the person outside the IDLH space enters to rescue, ensure a designee, once notified, would provide necessary assistance appropriate to the situation, and equip the employee outside IDLH space with pressure demand SCBA or other positive pressure supplied air with auxiliary SCBA, retrieval equipment that didn't increase overall entry risk or equivalent means to rescue when standard retrieval equipment is inappropriate.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: The employer is failing to protect employees from electrical shock, fire, or burn hazards. This was most recently documented at a work site located at 2416 Mid America Industrial Dr, Boonville, Missouri. Employees were exposed to serious injuries due to electrical shock, fire, or burns, in that electrical cord powering the Presto Lift was found to be in disrepair with damaged strain relief exposing outer insulation.
