29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about August  23, 2023, at 77 SE 5th Street in Miami, Florida, an employee was exposed to struck-by and crushing hazards while guiding a load that was rigged to a crane; the employee was not trained by the employer to recognize and avoid hazards associated with rigging, hoisting, and load guiding operations.
