29 CFR  1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed; and also were not required to travel with the load trailing whenever the load obstructed forward view:  (a) 100 Sawmill Road, Belleville, PA; at the loading dock - On or about August 25, 2023, employees operated powered industrial trucks with a load height of 104 inches, blocking the operators view.  The employer did not ensure that when the operator's view was block, the powered industrial truck traveled with the load trailing.        (b) 100 Sawmill Road, Belleville, PA; at the loading dock - On or about August 25, 2023, employees operated powered industrial trucks between the production areas and the loading dock. The employer did not ensure that when vision was obstructed the operators sounded the horn.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1910.178(q)(5): All parts of any such industrial truck requiring replacement shall be replaced only by parts equivalent as to safety with those used in the original design.   (a) 100 Sawmill Road, Belleville, PA - On or about August 25, 2023, the employer had the seat of the Harlo HP8500-4WD-16-85 Powered Industrial Truck serial #102343 replaced with an air-ride seat and did not ensure that the replaced seat was equipped with an seat belt.  No abatement or certification required.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  (a) 100 Sawmill Road, Belleville, PA - On or about August 25, 2023,  an employee suffered a work-related injury as a result of being struck by a forklift being operated by a another employee. The injury required hospitalization, and the employer failed to report this work-related hospitalization to OSHA within 24 hours. (The employer reported the in patient hospitalization to OSHA on Monday, August 28, 2023.)  No abatement certification or documentation required
