On or about August 22, 2023, employees were performing material handling work using a powered industrial trucks at a workplace located at 4140 Webster Street, Springfield, Missouri, where  they were exposed to struck-by hazards. Specifically, the employer failed to ensure that employees driving forklifts backwards were looking in the direction of travel. This failure resulted in an employee driving backwards, without looking in the direction of travel, colliding with the dust collection box located on the forks of another forklift. Subsequently, the operator of that forklift was crushed between the dust collection box and the mast of the forklift.
