At a jobsite located at 7232 NW Mace Rd, Kansas City, Missouri 64152, employees were exposed to eye and face injuries from flying particle hazards. Employees conducting residential roofing work were observed working with pneumatic nail guns without eye and face protection.

Employees conducting residential roofing work were observed working at elevations above 6 feet from a lower level without utilizing adequate fall protection systems.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a jobsite located at 7232 NW Mace Rd, Kansas City, Missouri 64152, employees were exposed to serious fall hazards. Employees conducting residential roofing work were observed accessing the roof by utilizing portable ladders. The portable ladder side rails did not extend at least 3 feet (.9 m) past the upper landing surface being accessed.
