29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 09/15/2023, in the Boiler Room, lockout procedures were not utilized when a boiler operator was servicing the boiler. The employee inadvertently reached into the rotary of the boiler while it was running to remove a buildup of sawdust when the blade came in contact with their finger resulting in a partial fingertip amputation.   ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On or about 09/19/2023, in the Boiler Room, employees that are required to Lockout machines such as, but not limited the boiler, when performing maintenance or servicing activities such as cleaning the rotary of the boiler, were not trained as authorized employees.   ABATEMENT DOCUMENTATION REQUIRED
