29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  a) Chipper Area; On or about September 18, 2023, and at times prior, employees were exposed to fall hazards up to 25 feet in that the employer did not train employees on fall hazards before they used a portable ladder.

29 CFR  1910.333(c)(7):Portable ladders did not have non conductive siderails if they were used where the employee or the ladder could contact exposed energized parts.   a) Sawmill Area; On or about September 18, 2023, and at times prior, employees were exposed to electrocution hazards in that the employer allowed employees to utilize a portable ladder without non-conductive side rails where the employee or the ladder could contact exposed energized parts.
