(dba: Clearview Seed), 2648 Benchland Road, Denton, MT: On or about September 23, 2023 and at times prior the employer did not report a work related fatality within 8 hours of the fatality occurring. The fatality occurred on evening of September 23, 2023 and OSHA was not notified until September 25, 2023 at 1:25 PM.

(dba: Clearview Seed), 2648 Benchland Road, Denton, MT:  On or about September 7, 2023 and at times prior the employer did not report a work related hospitalization within 24 hours of the injury occurring.  The event occurred on September 7, 2023 at approximately 3:30 PM and OSHA was not notified until September 25, 2023 at 1:25 PM.
