On or about 10/03/2023, at the site of 140-36 Negundo Ave. Flushing, NY  An employee removing roof shingles from a 3 and 12 pitched roof was not protected from falling approximately 24 feet by any type of fall protection system.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached:    A).	On or about 10/03/2023, at the site of 140-36 Negundo Ave. Flushing, NY  Two employees were each using a separate personal fall arrest system while working from a 3 and 12 pitched roof.  The roof brackets for each personal fall arrest system were fastened to the roof using only 3 of a possible 32 fasteners.

On or about 10/03/2023, at the site of 140-36 Negundo Ave. Flushing, NY  Employees utilizing a personal fall arrest system were not trained in the appropriate method of installing an anchor point. Only 3 out of 32 fasteners were used to secure the roof anchor bracket to the roof.

On or about 10/03/2023, at the site of 140-36 Negundo Ave. Flushing, NY  The employer did not report to OSHA the death of an employee as a result of a work-related incident.  An employee removing shingles from a 3 and 12 pitched roof, fell approximately 24 ft to the ground.
