29 CFR  1910.134(f)(2):Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:  a) Divine Rehabilitation and Nursing at Shane Hill located at 10731 State Route 118 in Rockford, Ohio;  On or about October 5, 2023, and at times thereafter, the employer did not provide a fit test to employees required to don N95 filtering facepiece respirators annually. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARs-CoV-2 virus while providing care such as, but not limited to, bathing, feeding, and dressing, residents suspected or confirmed positive with COVID-19.

29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Divine Rehabilitation and Nursing at Shane Hill located at 10731 State Route 118 in Rockford, Ohio;  On or about October 6, 2023, the employer did not ensure suitable facilities for the flushing of the eyes were provided where employees are exposed to corrosive materials. Employees handling NDC Morning Fresh Disinfectant Cleaner located in janitorial closets were exposed to hazards associated with corrosive liquids such as but not limited to severe skin burns and eye damage.

Employees were exposed to hazards associated with electrical shock when replacing 120 v electrical wall receptacles.
