29 CFR  1926.352(i): Drums, containers, or hollow structures which have contained toxic or flammable substances were not, before welding, cutting, or heating was undertaken on them, either be filled with water or thoroughly cleaned of such substances and ventilated and tested.   On or about October 7, 2023, the employer did not ensure that flammables were not present prior to welding on the storage tank. This condition exposed employees to an explosion hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about October 07, 2023, employees working on top of a storage tank 6-feet or more above a lower level did not have fall protection. This condition exposed employees to a fall hazard.
