29 CFR  1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  (a)  431 Patterson Bridge Road, John Day, Oregon 97845,Supervisor Building, on or about October 10, 2023 and at times prior thereto:  The Forest Supervisor did not ensure that fire extinguishers in the building were inspected monthly. This exposed employees to smoke inhalation hazards from potential delays from having a fire extinguisher not working.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.157(e)(2), which was contained in OSHA inspection number 1594183, notice number 1, item number 1a and was affirmed as a final order on8/18/2022, with respect to a workplace located at Forest Service, Parkdale Work Center.

29 CFR  1910.151(b): First aid supplies approved by a consulting physician were not readily available:  (a) 431 Patterson Bridge Road, John Day, Oregon 97845,Supervisor Building, Second floor, Break room,  on or about October 10, 2023 and at times prior thereto :  Employees were exposed to eye injuries from expired eye wash bottle solution located in the first aid kit.
