On or about October 13, 2023, at 1616 Penny Street, Tampa, FL. 33605, an employee was operating a forklift to lift/move palletized loads of equipment, during which time the forklift operator, while reversing the forklift, struck/backed-over an employee, wherein the forklift operator did not keep a clear view of the path of travel: therefore, exposing the employee to a struck-by hazard.

On or about October 13, 2023, at 1616 Penny Street, Tampa, FL. 33605, an employee was operating a forklift to lift/move palletized loads of equipment, during which time the forklift operator, while reversing the forklift, struck/backed-over an employee, wherein the forklift operator had not received adequate training in the topic of visibility including restrictions due to loading and forklift design/offset operators cab: therefore, exposing the employee to a struck-by hazard.
