29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about, October 16, 2023, the employer did not ensure that employees performing service and maintenance work on the paver plant screw conveyor were adequately trained on the methods and means necessary to isolate and control hazardous energy sources. Employees in the paver plant were exposed to caught-in and amputation hazards from the potential for unexpected energization and release of stored energy when performing service and maintenance work such as, but not limited to, cleaning activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d)(5)(i):Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. On or about, October 16, 2023, the employer did not ensure that stored energy in the control panel of the paver plant screw conveyor was discharged and/or controlled prior to allowing employees to perform service and maintenance work. As a result, employees working in the paver plant were exposed to caught-in and amputation hazards associated with the equipment's rotating screw auger while performing service and maintenance activities such as, but not limited to, cleaning activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and de-energization of the machine or equipment have been accomplished. On or about October 16, 2023, the employer did not ensure that employees verified isolation and de-energization of hazardous energy sources prior to allowing them to perform work on the paver plant screw conveyor. On or about October 16, 2023, the employer failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the components of the tiger plant screw conveyor in order to safely de-energize the equipment and control the hazardous electrical energy. Energy control procedure TGP-WS-C lacked specific steps for discharging stored electrical energy contained in the control panel prior to equipment access. Additionally, clear and specific verification steps were not prescribed to determine the effectiveness of the energy control measures. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
