29 CFR  1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) Downstairs Storage: On or about October 13, 2023, and times prior, the portable fire extinguisher located in the downstairs storage area was not mounted, located and identified so that it may be readily accessible to employees. The extinguisher was being stored on the floor next to the desk area.  b) Primary Kitchen: On or about October 13, 2023, and times prior, the portable fire extinguisher located in the kitchen area was not mounted, located and identified so that it may be readily accessible to employees. The extinguisher was being stored on the floor under the sink.  c) Congregate Area: On or about October 13, 2023, and times prior, the portable fire extinguisher located in the upstairs congregate area was not mounted, located and identified so that it may be readily accessible to employees. The extinguisher was being stored on the floor in the closet.  d) Secondary Kitchen: On or about October 13, 2023, and times prior, the portable fire extinguisher located in the secondary kitchen area was not mounted, located and identified so that it may be readily accessible to employees. The extinguisher was being stored on the floor next to the exit door.

29 CFR  1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) Downstairs Furnace Room: On or about October 13, 2023, and times prior, an extension cord was used to provide power to the sump pump. The extension cord was being used in lieu of permanent wiring.

29 CFR  1910.305(g)(1)(iv)(C):Flexible cords and cables were used where run through doorways, windows, or similar openings.  a) Downstairs Furnace Room: On or about October 13, 2023, and times prior, the flexible cord of a trouble light was ran through a doorway connecting the downstairs storage area to the furnace room. The cord was pinched in the closed doorway exposing the cord to potential damage.
