29 CFR  1910.212(a)(1): Machine guarding - Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about October 12, 2023, the employer failed to ensure the HAAS VF-2 Mill (Serial No. 1161168) was properly guarded in that the plexiglass doors used a barrier that had an interlock system that had been altered to allow the machine to run while they were open. As a result, an employee suffered an amputation after being exposed to rotating parts.  b) On or before October 17, 2023, the employer failed to ensure that the MORI SEIKI SL-35B (Serial No. 1293) was properly guarded, in that the door used as a barrier had an interlock system that had been altered to allow the machine to run while they were open. As a result, employees were exposed to rotating parts and hazards such as, but not limited to caught-by and amputation.
