A) 97 Roxy Ave Edison, NJ 08820 Employees were working off pump jack scaffold that was not properly secured to the structure. The scaffold was only secured at the top of the pole. This condition exposed employees to fall hazards of approx. 12 feet. Hazard noted on or about 10/24/23
