29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 10/25/2023, the employer did not protect employees who were engaged in residential roofing operations 6ft (1.8m) or more above lower levels while at the job site located at 2521 Bear Creak Dr., Wentzville, MO. The employees were conducting residential roofing operations and were exposed to the hazard of falling approximately twenty-two (22) feet.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about 10/25/2023,the employer did not ensure portable ladders were constructed and placed so three (3) feet or more were extended above the upper landing at a job site located at 2521 Bear Creak Dr., Wentzville, MO. Workers performing residential construction were exposed to falling approximately twenty two (22) feet while using a straight ladder to ascend/descend from the ground level to the roof.
