29 CFR  1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:   A.) Plastek Industries Inc. DBA Plastek Group, Warehouse, On or about 10/11/23 and times prior thereto: an employee operated Clark L-43 forklift while not keeping a clear view of the path of travel. The forklift made contact with a pedestrian.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Plastek Industries Inc DBA Plastek Group, Warehouse, On or about 10/30/23 and times prior thereto: long fork attachment was being used with equipment including but not limited to, RC 3000 Series Forklift Serial Number 1A229300.  The long forks are used to move long boxes. The employer had not received permission from the forklift manufacturer to use the attachment.    ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  A.)	Plastek Industries Inc DBA Plastek Group, Warehouse, On or about 10/30/23 and times prior thereto: long fork attachment was being used with equipment including but not limited to, RC 3000 Series Forklift Serial Number 1A229300.  The long forks are used to move long boxes. The employer had not obtained a new forklift plate to identify the attachment and changes in lifting limitations.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
