29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) This violation occurred on or about November 03, 2023, at a workplace located at 2929 Avicenna St., Irving, TX; where employees, who were working under construction activities, were not protected from head injuries by wearing hard hats.  The Lone star Green Homes, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1477939, citation number 1, item number 1 and was affirmed as a final order on 08/20/2020, with respect to a workplace located at 2933 Avicenna St., Irving, TX 75062; And  The Lone star Green Homes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1676125, citation number 2, item number 1 and was affirmed as a final order on 10/11/2023, with respect to a workplace located at 4626 Ghazali St., Irving, TX 75062.
