29 CFR 1926.21(b)(2):  The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a) SRPACK Machine - On or about November 3, 2023, employee(s) were exposed to entanglement injuries while working at the feed side of the machine. The existing guarding did not prevent employees from having fingers and arms pulled into the danger zone to the right and left of the cardboard feed area.
