29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   13015 FM 1957 Potranco Road, San Antonio, TX 78245: On or about November 8, 2023, and at times prior thereto, employees were exposed to a fall hazard while working in a man-made basket while being lifting by a CAT Model TL642C that was not approved by the manufacturer.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: Note:  Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift:    13015 FM 1957 Potranco Road, San Antonio, TX 78245: On or about November 8, 2023, and times prior thereto, employees were exposed to fall hazards while riding inside a man-made personnel work platform attached to a CAT Model TL642C Frontend Loader that was not secured to the forks.
