29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) Lot 469 Del Webb at Chateau Elan subdivision, Hoschton, GA: On or about 11/14/2023, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that protective eyewear was not provided to employees using these tools at 120 PSI while engaged in roofing activities.

29 CFR  1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection.   a) Lot 469 Del Webb at Chateau Elan subdivision, Hoschton, GA: On or about 11/14/2023, the employer exposed employees to 9-foot fall hazards while performing roofing operations, in that ropes were tied into knots, connected to the trusses as anchor points.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   a) Lot 469 Del Webb at Chateau Elan subdivision, Hoschton, GA: On or about 11/14/2023, the employer exposed employees to 9-foot fall hazards while performing roofing operations, in that the employer allowed for the use of rock-climbing type harnesses and not full body harnesses as the connection to the harness was located near the waist and not middle of the wearer's back.
