29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) Forge Signworks 69 Megil Road Farmingdale, NJ 07727: On or about 11/15/2023, the employer did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i) - (ix). The employer required employees such as Grinders, Welders, Helpers and Painters to wear 3M 6200 half face elastomeric respirators, and GVS Elipse half face respirators, throughout their shifts while grinding. welding or painting materials.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

The employer requires Painters, Grinders, Helpers, and Welders to wear 3M 6200 half face elastomeric respirators, and GVS Elipse half face respirators while spray painting, grinding, and/or welding.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
