29 CFR  1910.147(c)(5)(ii): Lockout devices and tagout devices were not singularly identified; were not the only devices used for controlling energy; were used for other purposes; and did not meet the requirements of subparagraphs (A) through (D):   On or about: November 20, 2023 Location: 615 Ferry Street, Newark, NJ 07105  a) The employer did not ensure that specific lockout devices were singularly identified when using lock to lock out pneumatic pressure for 121-M-6 machine.  All employees used the same lock to perform LOTO for pneumatics, which was located at the machine.

29 CFR  1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  On or about: November 20, 2023 Location: 615 Ferry Street, Newark, NJ 07105  a) 480 Volts cabinet for 121-E9 Allen-Bradley Feed Master Machine, was not securely closed, exposing employees to electrocution hazards by accidental contact.
