29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about November 21, 2023, and dates prior, at the Mazak Quick turn Nexus 200 MY CNC Lathe (L3), the employer did not insure the door interlock was engaged and operated properly to prevent employee exposure to the point of operation of the CNC lathe, to include the energized machine spindle (chuck) and tooling axis arm. This exposed employees to a struck-by and caught-in hazards.
