On or about November 17, 2023, the employer failed to ensure that the working surface in front of the mobile platform ladder was clean and orderly. An employee utilizing the mobile platform ladder was unable to get close enough to the rack and overreached the rail to retrieve supplies thus causing the employee to lose balance and fall to the ground below.
