29 CFR 1910.219(c)(4)(i): Shaft ends projecting more than one-half the diameter of the shaft were not guarded by nonrotating caps or safety sleeves:   Employees in the Dried Fertilizer Building were exposed to caught-in hazards.  Employees engaged in cleaning up a pile of fertilizer activities were exposed to caught-in hazards of rotating parts in that a 1.5 inches diameter rotating shaft end of the load out conveyor located at 4 feet 3 inches above the floor projected 3 1/4 inches was not guarded.

29 CFR 1910.178(a)(6): Nameplates or markings for powered industrial trucks were not in place or were not maintained in a legible condition:  Employees were exposed to struck-by hazards.  Employees engaged in material handing activities using the following forklifts were exposed to stuck-by hazards in that the following markings were not in place and legible: a) Chemical Storage Building - Komatsu forklift, model FG20C-12, serial number 510136 - The side shift control lever was worn out, replacing with a new one did not include markings. b) Chemical Storage Building - Hyundai forklift, model 25D-9, serial number HAKHHN12PL0001365 - The side shift control lever did not include markings. c) Seed Storage Building - 2022 new Hyster forklift, model H50A, serial number R177BO3650X - The side shift control lever did not include markings. d) Seed Storage Building - Hyundai forklift, model 25D-9, serial number HHKHHN12KL0001366 - The side shift control lever did not include markings.
